CARE MINISTRY

Our Community Care Ministry is focused on a practical approach to meet the physical and spiritual needs of the whole person. One way we are called to fulfill this mission is by helping those who are in need, including financial counseling, limited monetary assistance, resource referrals, and practical assistance in a variety of other areas.

 

If you need financial assistance, we now require you first visit your local Trustee’s office before coming to Community Care at Chapel Rock. If they are unable to help you, please bring your letter from the Trustee indicating they were unable to assist you when you come to Community Care along with the documents listed later below.

  • The third Monday of each month has been set aside to meet with you and have a conversation to understand your situation. Please enter door #3 on the south side of the building between 6:00 and 7:15 p.m.  Please do not enter early as our team will be preparing. No appointment is necessary. 

     

    Once you have been interviewed and have had an opportunity to explain your situation, the Community Care Ministry leadership will review your request(s). The following day a staff member will contact you to inform you whether or not we can assist you financially. Regardless of whether you receive financial assistance or not, you will not be able to be seen again for three months (90 days).

     

    Please be aware we have limited financial resources and unfortunately may not be able to assist every household financially. We do not have cash on hand during the meetings. If it is determined you will receive financial assistance from Chapel Rock, it will be in the form of a check, made payable to the utility or paid via the phone from our offices. 

  • • Driver’s license or government-issued identification card

    • You must bring with you any invoices, electric bill, gas bill, etc. We cannot process any payment without your “support” materials/paperwork. We do have the capability of making copies on site.

    • Invoices must be in the name of the person requesting assistance.

    • It is acceptable to email copies of your invoices to careteam@chapelrock.org BEFORE 3:00 p.m. on the day Community Care Ministry meets. Copies must include your name and address, the full account number, and the amount due. We are unable to use invoices on your phone for consideration. If we have not received your invoices by email before Community Care Ministry begins, we will be unable to assist with your need.

    • Dates of birth for all persons living in your household.

    • You will be asked to list your monthly income total and expenses. Please bring any paperwork that will help substantiate your income / expenses.

  • Please note our Care Ministry will be able to assist only families which live in the following ten zip codes.

  • Connect 2 Help Hand Holding Heart

    If you’re not familiar with 2-1-1, think of it as 9-1-1 for social service needs. Connect2Help 2-1-1 provides information and assistance with food, utilities, aging issues, counseling and finding safety from violent situations.  They are your primary source for community, health, crisis, and disaster information. Their staff is trained and dedicated to providing a caring, professional source of guidance and advocacy. They are your bridge to a better tomorrow. 2-1-1: It’s easy to remember, easy to use, free, confidential, and available 24/7. You have a place to call for help whenever you or a loved one needs it.
